Community Support

 
 

 

Mission: The community support program aims to promote a sense of belonging within a community, to support the autonomy of a person within that community and to provide assistance in the realization of these aims. The person's right to make personal choices and decisions is respected within the context of appropriate guidance.

Some individuals progress through support services very quickly and require minimal on-going follow-up. Others may require close follow-up for the rest of their lives.

Objectives:

  • to assist individuals in their functional autonomy, i.e. individual budget plans, apartment management;
  • to assist individuals in identifying vocational goals and provide guidance in locating vocational resources;
  • to provide ongoing follow-up, support and counselling to individuals living in their own apartments;
  • to provide general advocacy, and to promote the development of links with groups which promote social development;
  • through yearly individual goal planning, to help individuals identify their personal goals
  • to provide individual counselling to individuals who need support in personal matters.

General description: Community support services offered include:

  • Individual follow-up and support for clients living on their own. This can include weekly contact with clients (including home visits), assistance with menu planning, groceries, budget programs and apartment management skills. Counselling is provided for vocational and personal issues.
  • More intensive follow-up of clients living in our designated supervised apartment(s). This usually involves a minimum of two contacts per week with a greater emphasis on direct assistance and providing a high level of support.
  • Some community support clients need minimal practical assistance with their apartment living skills. Interventions may focus on counselling around personal issues and vocational goals as well as acting as liaison with various community resources and providing a general advocacy role.

    Individuals receiving community support services have access to our social development services as well as our community education program.
