AVATIL’s community support services are part of the long-term support services AVATIL provides to clients living in apartments in the community.

The community support program aims to support the autonomy of individuals living in the community and to promote a sense of belonging within a community.

The person’s right to make personal choices and decisions is respected within the context of appropriate guidance.  Different levels of community support are provided depending on the needs of the individual.

The services provided include practical assistance with independent living skills in areas such as budgeting, apartment management, menu planning and grocery shopping. AVATIL also provides accompaniment to health care appointments and provides a liaison with health professionals and home care services, particularly on behalf of older clients so that they can continue to live in their apartments as long as possible.

Support around personal matters is provided as well as resource linking and general advocacy.

Family meetings are held with clients receiving community support services every 1 – 2 years.  Goals are identified by clients in areas such as residential, health, vocational, social and finances.  These goals are reviewed with their AVATIL counselors every 3 months.

Individuals receiving AVATIL community support services also have access to social development services.
